PLEASE BRING A VALID OHIP / HEALTH CARD TO EVERY VISIT!
To receive health care services covered by OHIP in the province of Ontario, ALL PATIENTS must bring a valid Health Card to medical appointments. Our office will ask you to produce your Health Card at every visit. We do not accept pictures or photocopies of Health Cards.
If your Health Card is invalid (due to change of name, change of version code, or expiry), or you do not have your Health Card with you, we will charge you for your visit at regular OHIP rates before you are seen. We will refund the full fee upon presentation of a valid Health Card.
If your Health Card has expired it is your responsibility to visit a Service Ontario location to renew the card. Lost, stolen or damaged Health Cards should be reported to the Ministry of Health and Long Term Care at 1-800-664-8998. For more information, please visit Service Ontario’s website.
